Cell-Up Application Form

  1. All phone packages include both a used or new smartphone, charging cable and power brick, and 1-3 months of pre-paid cellular service.

  2. Account passwords and login information will be provided via email to the primary contact listed below. It is the responsibility of the contact person to provide the account information to sponsored clients.

  3. After Connect Vancouver support expires, clients can continue to pay for their account online, or by purchasing cell ‘top-ups’ at available retailers. The payment per month before taxes is approx $25.00 CAD for most plans.

  4. Packages are created for clients without an existing line or cellular device. Connect Vancouver cannot provide cellular service or smartphones in isolation (they are provided together in each package).

  5. Merging existing phone lines are not possible under this program.

  6. If you have a client seeking only a smartphone, please contact us separately to be placed on a waitlist. Phones not used for this program will be donated following the completion of this program in August.

  7. This form is for triaging confirmed support requests. Please do not share this form outside of your organization.

  8. Due to volunteer shortages, current wait times for phone drop-offs is between two to four weeks from the time of form submission. Our team will contact you once the phone package(s) are ready for drop-off.
